Welcome to the Diamond Lake School District Health Services website!
On this website you will find all state health forms needed to meet the state’s various health requirements. The forms are downloadable so you can print them off at home.
The health requirements are as listed below:
Pre-K/Early Childhood: IL Physical and appropriate immunizations
Kindergarten: IL Physical and appropriate immunizations, dental exam and a vision exam
2nd Grade: Dental exam
6th Grade: IL Physical and appropriate immunizations, dental exam
All incoming students from another state or country are required to have: IL Physical and appropriate immunizations and a vision exam

Flu Season!
As of August 2009, the Lake County Health Department is recommending that any child with a fever over 100 F and one of the following symptoms, cough, sore throat or body aches to stay home from school until fever-free for 24 hours without taking a fever-reducing medication.
Please keep the nursing and attendance staff informed of the type of illness your child has. Our job is to keep everyone healthy!
The best defense is to cover your sneezes and coughs and wash you hands frequently, especially after coughing sneezing and going to the bathroom.

Please keep your contact information up to date. In the event of an illness or injury, the nurses need to be able to contact you quickly. Let the office staff or nurses know as soon as possible.
PLEASE KEEP YOUR CHILD HOME IF THEY HAVE BEEN VOMITING, HAVING DIARHEA OR HAVE A FEVER OVER 100 F IN THE LAST 24 HOURS! This will help everyone stay healthy.
